Professional Waiver
Non-Professional Counseling & Liability Waiver
I acknowledge that the method and process by which this advice and direction are given in no way constitutes an agreement or liability
on the part of Graceful and is different than a one-on-one clinical or psychological counseling, and that this program is not meant to be a substitute
for clinical or psychological counseling.
In consideration of my participation in the program and to the fullest extent permitted by law, I hereby expressly release and agree to hold harmless
Graceful, its officers, directors, employees, agents, volunteers, and assigns from and against any and all claims, actions, disputes, judgments or expenses arising out of, or incident to my participation in the program.
Assumption of Risk & Legal Force
"Claim" as used in this agreement means any financial loss, claim, suit, action, damages, or expense, including but not limited to attorney's fees, attributable to
any loss resulting therefrom.
I further understand that this assumption of risk and hold harmless is intended to be as broad and inclusive as permitted by the laws of The State of Florida and that if any portion hereof is held invalid, I agree that the balance shall, notwithstanding, continue in full legal force
and effect.
